What is hospital case management?
Our department consists of registered nurses and a licensed social worker who work closely with your physician to coordinate hospital services while ensuring quality of care and promoting cost effective outcomes.

Who receives case management services?
All inpatients are assessed by a case manager on the first full business day of their stay.

What services will my case manager provide?
Your case manager will initially review your chart to assess your home environment, your support system and your plans for post-hospital care. She will assist you and/or your family with arranging any post-hospital needs such as inpatient or outpatient rehab, home health, hospice care or skilled nursing facility care. She can advise you on Medicare or Medicaid regulations, medication assistance programs, community service programs and insurance coverage limitations.

Will I have the same case manager throughout my hospitalization?
No. Our case managers are unit-based. If you are transferred to a new nursing unit, a different case manager will assume your care.

How can I contact my case manager?
We are available to talk with you or your family Monday through Friday 8:00 a.m. to 4:30 p.m. Your nurse can phone your case manager at your request. Your family can reach the case management department at 903-531-8138. However, we cannot provide information to your family without your permission.

Will case management help with obtaining financial assistance?
We will assist in connecting you with a third party financial counselor who can assist you with information about disability and state-funded insurance programs.
