General patient registration form
These general guidelines usually apply to urological surgery and procedures when an inpatient stay is required.Your physician may give you more instructions about preparing for treatment.
If you are scheduled for surgery, your physician will write an order for nothing by mouth. This usually begins at midnight prior to the day of surgery. The nurses will inform you not to eat or drink after a certain time.
Before your surgery:
- Have on a clean hospital gown
- Remove all jewelry.
- Give valuables to family members or store them in the hospital safe in the admitting office.
The nurse will go over a pre-op checklist to ensure you are prepared.
Approximately one hour prior to your scheduled surgery time, you will be transported to the holding department located in the basement. Due to limited space, only one family member is allowed to go with you to this area. The staff on this unit will review your chart and prepare you thoroughly for your procedure.
Your family will need to come back to the nursing unit when you are moved to the surgical area. The operating room and post anesthesia care units will notify your family by telephone about your condition. Normally, recovery room time is approximately one hour; however, this may vary depending on the administration of medication.
Upon your return to the nursing unit, the nursing staff will be obtaining frequent vital signs (blood pressure, pulse, respiration rate and temperature) and informing you of the new postoperative routine. It will be very important for you to turn, take deep breaths and cough every 2 hours (unless otherwise instructed). You will need to inform the staff of any discomfort you may have. Normally, you are allowed to be up on your feet the day following surgery.
The hemodialysis staff will transport you from your room to the dialysis unit. There is not a daily schedule for treatments due to emergencies, ICU patients, etc. There is a visitor's area in the dialysis unit.
Press 9 for local calls, then dial your number.
Long Distance The hospital operator will be glad to assist you or you may use your calling card by pressing 9, then 0 and proceed as directed on your calling card.
Collect Calls Dial 0 and give the information to our operator.
Room Number Your telephone number in your room will be 531 plus 8 and the last 3 digits of your room number, except for 2230B which is 531-8536 and 2243B which is 531-8537. If you have any problems, please ask for assistance from the staff or the operator.
Please note: Your telephone receiver must be lying on a flat surface to be ready for incoming calls.
The physicians usually will see patients in ICU first and then come to the floor and see patients at various hours. You may see your doctor at the unit several times during the day. Your physician will see you in your room, not at the desk.
The radiology department is notified when an order is written for an X-ray. The radiology department performs the X-ray (keeping in mind the needs of all patients in the hospital) in the most timely fashion possible. Therefore, most X-rays are not scheduled for a certain time of day. (Some X-rays require preparation and must be done the next day.)
Your meals are served between the hours of 6:30 a.m.-7:30 a.m.,
11:30 a.m.-12:30 p.m. and 5:30 p.m.-6:30 p.m. Your physician will order your diet. You may dial MENU (6368) from your room phone to inquire about what is being served or to request a special food item for a meal allowed on your diet.
A hostess will bring your tray into your room and the floor staff will assist in setting it up for you. Please do not hesitate to push your call light if you need assistance.
Fluid management/Daily weights
You may be asked to record the amount of everything you drink and the amount of urine you produce. Gentlemen, please use the urinal provided. Ladies, please use the white hat-shaped container provided on the toilet. When you tell the staff that you have some urine to be measured, they will note the amount and discard the urine for you.
You may be weighed early each morning. We apologize for the early wake-up call, but weights at other times of the day are not as informative.
Offering/obtaining magazines and newspapers;
You will be offered a bag bath or a shower, if allowed. The bag bath is a single step procedure. The staff will bring a plastic bag into your room filled with warm washcloths. You will use a special washcloth for each extremity. It is not necessary to dry off or apply a lotion afterward.
PALSPeople Applying Loving Services
PALS represent an unlicensed person who is on hand in our units to assist you with many activities that do not require a licensed employee. PALS helps deliver information to you or to our staff concerning your care. Some other examples of how you can use our PALS or what you may see our PALS doing are as follows:
Writing and/or mailing letters;
Offering water or coffee to patients and families;
Serving as someone to talk to;
Assisting with the patient's luggage if it is on another unit;
Assisting with phone calls;
Assisting with combing hair;
Assisting with discharge;
Reporting your needs to nurses;
Making sleeping arrangements for your family;
Orientation of your room.
Please allow our PALS to assist you with these needs during your stay.
Ideally, your discharge planning should be discussed prior to your admission to the hospital. The case manager will assist with any discharge needs that the physicians feel you require (for example, equipment, home health, rehabilitation or nursing home).
Please let us know your concerns regarding discharge as early as possible so that everything will be ready for you when you are discharged.
After your physician informs you of the discharge, your nurse will complete the discharge teaching and paperwork. The nurse will come to your room and prepare you for discharge. You will be accompanied to your vehicle by the nursing staff.
