A rehabilitation stay is usually followed by outpatient therapy visits, regular home exercise programs or home health agency follow-up. This will be discussed with team members prior to discharge. Again, if you have a preference in regards to an outpatient clinic or a home health agency, please let it be known. Any comments or suggestions about the rehabilitation program will be welcomed either during or after an inpatient stay.
Any medical equipment needs will be addressed during the rehabilitation stay. Equipment can be ordered and delivered prior to discharge from rehabilitation-either to the patient's home/destination or to the hospital. Any equipment that is covered by or reimbursed by insurance carriers will require a physician's prescription, which will be obtained prior to discharge from the unit.
In addition, a staff member of the rehabilitation hospital will attempt to contact you by phone following your discharge from the program. This continued interaction is important in obtaining feedback and evaluation and documenting your continued progress.
